Informed Consent for Database Survey

Description of Research
You are invited to take part in a research study called Database Survey. The study is designed to
learn more about the participants within our participant database. The research is being
conducted by Dr. Tessa West, who is a faculty member in NYU’s Department of Psychology. If
you decide to participate in this research, you will be asked to answer background questions
about yourself (e.g., age, gender identity, health information, etc.), enter your contact
information, and indicate what types of research studies you would be interested in participating
in. Participation in this study will involve 5 minutes of your time.
What are the risks and benefits of participation?
There are no known risks associated with your participation in this research beyond those of
everyday life. It is possible that you will reveal some potentially identifying or sensitive
information on the survey. We take every precaution to protect your confidentiality and
minimize the risk of a breach (see confidentiality section below). Taking part in this study is
voluntary. You may decline to participate or withdraw at any time without penalty. Participation
in this study may result in opportunities for you to participate in future research studies.
How will my confidentiality be protected?
Confidentiality of your research records will be strictly maintained by storing any data
containing identifying information in a secure, password-protected database which only the
research team will have access to. Information not containing identifiers may be used in future
research, shared with other researchers, or placed in a data repository without your additional
consent.
